Who are we?
On September 15, 2001, a group of dedicated individuals in Tafelkop village, both women and men, united with a common purpose: to alleviate the suffering of chronically ill residents. These compassionate women, representing various sub-villages within Tafelkop, came together to establish the Tafelkop Lesedi Home Community Based Care, a non-profit organization committed to making a positive impact.
Tafelkop Lesedi Home Based Care: Empowering Communities Through Compassionate Initiatives Since its establishment in 2001, Tafelkop Lesedi Home Based Care has been a beacon of hope for the residents of Elias Motswaledi and neighboring municipalities in Sekhukhune. Our unwavering commitment to comprehensive health and social support has garnered recognition and support from the Department of Social Development (DSD) NPO Directorate.

Key Milestones:
- Founding and Registration (2003): On September 15, 2003, we officially registered as an organization dedicated to providing holistic care. Our mission extends beyond medical assistance; it encompasses emotional support, education, and advocacy.
- Healthcare Outreach (2005): Recognizing the urgent need, we initiated community-based efforts to combat HIV/AIDS, tuberculosis, and other health challenges. Our tireless work earned the respect of the Department of Health and Social Development, leading to a fruitful partnership and essential funding.
- Income Generation and Community Services:
- Sewing Project: Our income-generating sewing project empowered local artisans and contributed to sustainability.
- Infrastructure Support: We acquired essential resources, including tents, toilets, and chairs, enabling us to serve the community effectively.
- Home Community Nursing Care: Our team of 24 dedicated women provided compassionate care to terminally ill patients, emphasizing medication adherence and emotional well-being.
- Thabantsho Community Radio Partnership: Through this collaboration, we disseminated vital information on substance abuse, immunization, COVID-19, and other social challenges.
- Job Creation and Empowerment (2019–2022):
- Covid-19 Ambassadors: We employed 400 vulnerable individuals, including youth, women, men, and those living with disabilities, to serve as frontline ambassadors during the pandemic.
- Environmental Cleaners: Our team contributed to maintaining a clean and safe environment.
- Education Support: School and clinic assistants played a crucial role in supporting education and healthcare services.
- ECD Centers: We provided essential support to early childhood development centers.
- EPWP Recognition: Our organization was honored as the best recruiter of youth, women, and people with disabilities in Limpopo Province during the EPWP Colloquium.
- Covid-19 Health Education: We engaged 100 matriculants and grade 10 graduates, equipping them with knowledge to combat the pandemic. Funding from the Independent Development Trust (IDT) facilitated this critical initiative.
